It also includes the 12-digit Pag-IBIG Membership ID (MID) Number as well as the Registration Tracking Number (RTN).

These days, registering as a member of the Pag-IBIG Fund can be done online. Prospective applicants need not fill out and submit the Pag-IBIG MDF because they can simply input their information on the Pag-IBIG online registration website. This is definitely easier and more convenient than the old method of manually filling out forms and personally bringing them to the Pag-IBIG office.

However, you can still use the Pag-IBIG MDF to apply for a Pag-IBIG number if you can’t (or don’t want to) register on the Pag-IBIG website. The Pag-IBIG Fund still accepts applications this way. Just download the Pag-IBIG MDF and print it on short bond paper, or you can get one at the Pag-IBIG office nearest you.

How to Get Pag-IBIG MDF for Existing Members

At the moment, there is no option to request your Pag-IBIG MDF online if you’re an existing member. All requests for a copy of the form must be performed personally at the nearest Pag-IBIG Fund office.

To request a hard copy of your Pag-IBIG MDF indicating your Pag-IBIG MID number, just visit any Pag-IBIG branch and bring the following:

• Original and photocopy of valid ID acceptable by the Pag-IBIG Fund, and
• Duly accomplished Pag-IBIG Fund Request Slip (HQP-PFF-369).

You may download the Request Slip here. After downloading the slip, open the PDF file and print it (press Ctrl + P) on long bond paper (8″ x 13″). If you don’t have long bond paper, you may print it on short or A4 paper as long as you set the size to fit.

1. Requestor’s Name – Write your (or the Pag-IBIG member’s) complete name.
2. Date of Release – You may leave this blank.
3. Pag-IBIG MID No. – Indicate your 12-digit Pag-IBIG Membership ID number. If you forgot or lost your Pag-IBIG MID number, read this article to learn how to recover it.
4. Employer ID No. – Specify the Pag-IBIG Employer ID number of your current employer as stated in the Employer’s Data Form (EDF).
5. Purpose – Write the purpose of your request. Example: “For employment.”
6. Particulars – In the Particulars column, check the box for “Member’s Data Form (MDF, system-generated).”
7. No. of Copies – In this column, indicate how many copies of the Pag-IBIG MDF you want.
8. Requested by – Affix your signature over your printed name.
9. Date – Write the date the request was made.

After accomplishing the Request Slip, go to the nearest Pag-IBIG branch and submit it along with a photocopy of your valid ID. You must also bring your original ID for authentication purposes.

Pag-IBIG branches are open from 8:00 AM to 5:00 PM, Mondays to Fridays (except for holidays).

Your requested Pag-IBIG MDF with Pag-IBIG MID number will be released on the same day. Your authorized representative may also claim your Pag-IBIG MDF by presenting an authorization letter and valid IDs of the requestor and the authorized representative.

Take note that the Pag-IBIG MDF that you will receive is a system-generated copy, not the original form that you submitted during membership registration.

Select the appropriate option to indicate the type of membership coverage under Republic Act No. 9679.

• Mandatory – Refers to membership which is required or compulsory.
  • Employed (Private) – Check this box if you are working for a private employer, regardless if you’re already registered with the Social Security System (SSS) or not. This includes foreign employers with an administrative agreement with the Pag-IBIG Fund.
  • Employed (Government) – Check this box if you are working for a government office that is covered by the Government Service Insurance System (GSIS). This includes uniformed personnel of the Armed Forces of the Philippines (AFP), Philippine National Police (PNP), Bureau of Fire Protection (BFP), and Bureau of Jail Management and Penology (BJMP), as well as members of the judiciary and Constitutional Commissions.
  • Employed Private Household – Check this box if you are working in a private household (e.g. domestic helper).
  • Overseas Filipino Worker (OFW) – Select this if you are working for a foreign-based employer, whether based on land or at sea.
  • Self-Employed – If you are engaged in a business, trade or profession with a monthly income of at least ₱1,000, not over 60 years old, and not under an employer-employee relationship, check this box and then select the corresponding sub-category below.
    • Professional/Business Owner – Check this box if you are earning income through your own business or profession.
    • Job Order Personnel – Check this box if you are hired to perform a specific task or short-term work of less than 6 months, and you are paid on a daily or hourly basis.
    • Other Earning Group (OEGs) – This refers to small scale units that produce goods and services meant to generate income. Examples of OEGs are farmers, fishermen, public utility transport drivers, and market vendors.
    • Employed (Foreign Government) – This refers to employees of foreign governments (embassies and consulates) or international organizations without an administrative agreement with the Pag-IBIG Fund.
    • Barangay Official/Employee – Check this box if you are a person of authority in your barangay.
    • Non-Working Spouse – Check this box if you are a spouse (husband or wife) who is devoted full-time to managing the household and family affairs.
    • Member of Religious Group – Check this box if you are a member, head or leader of an organization in the exercise of religious belief.
    • Pensioner/Investor/Lessor – A pensioner is a person receiving old-age or permanent total disability pension, or has received the lump sum. An investor is the owner of securities or shares including shares of stock issued by a Real Estate Investment Trust (REIT) or its derivatives. A lessor is the owner, administrator or agent of a residential unit.
    • Member of Cooperative/Trade Union – Check this box if you are a member of a cooperative or a trade union.
    • Overseas Filipino Immigrant – If you are a Filipino who lives outside of the Philippines as a citizen or as a permanent resident of another country, check this box.

    • Member – Accomplish this field in the following order:
      • Last Name – Write your surname or family name.
      • First Name – Write your given name.
      • Name Extension – If your name includes a suffix (e.g. Jr., III, IV), please indicate it here.
      • Middle Name – This is usually your mother’s maiden surname, and in the case of married women, your father’s surname.

      • Permanent Home Address – Write the complete address of your permanent residence including barangay, municipality/city, province and ZIP code. Only include the state/country if home address is outside the Philippines.
      • Present Home Address – Write the complete address where you currently reside. Only include the state/country if present address is outside the Philippines.
      • Preferred Mailing Address – Check the appropriate box to indicate the address where mail will be sent.
      • Contact Numbers – Provide your home telephone number, cellphone number and business phone number (if any). Include the country code and area code only if you’re outside Metro Manila or based abroad.
      • Email Address – Write your email address (Gmail or Yahoo! Mail).

      Present Employment Details

      • Occupation – Indicate your job, profession or line of work (e.g. Teacher).
      • Employment Status – Check the appropriate box with your employment status.
      • Employer/Business Name – Write the complete name of your employer or business based on the registration certificate or employment contract (applicable for formally employed, OFWs, self-employed professionals and business owners).
      • Employer/Business Address – Write the complete address of your employer or business based on the registration certificate or employment contract.
      • Type of Work – This field is for OFWs only. Check the appropriate box for your type of work, whether land-based or sea-based, and then specify the country of assignment.
      • Monthly Income – Indicate your income or earnings per month, including basic salary/income and allowances.
      • Office Assignment – Check the appropriate box on whether you’re assigned to the head office or a particular branch. If you’re assigned to a branch, write its location on the space provided.
      • Date Employed – Write the month and year when you started working for your present employer.

      Previous Employment From Date of Pag-IBIG Fund Membership

      • Employer/Business Name – Write the complete name of your employer or business based on the registration certificate or employment contract (applicable for formally employed, OFWs, self-employed professionals and business owners).
      • Employer/Business Address – Write the complete address of your employer or business based on the registration certificate or employment contract.
      • Office Assignment – Check the appropriate box on whether you’re assigned to the head office or a particular branch. If you’re assigned to a branch, write its location on the space provided.
      • From – Indicate the month and year (MM/YYYY) when you started work for the particular employer or business. For example, if you started work on July 2020, write 07 2020.
      • To – Indicate the month and year (MM/YYYY) when you ended your work with the particular employer or business.

      Heirs

      List your legal heirs in accordance with the Laws of Succession, as provided in the New Civil Code of the Philippines (as amended). You can use another sheet if necessary.

      • Name
        • Last Name – Write the surname or family name of your heir.
        • First Name – Write your heir’s given name.
        • Name Extension – If the name of your heir includes a suffix (e.g. Jr., III, IV), please indicate it here.
        • Middle Name – This is usually the mother’s maiden surname, and in the case of married women, the father’s surname.
        • No Middle Name – Check this box if your heir doesn’t have a middle name (e.g. illegitimate child).

        Certification

        Read the certification carefully, and then affix your signature and indicate the date when the Pag-IBIG MDF was accomplished.

        • Signature of Informant – Affix your signature on the space provided.
        • Date – Write the date when you accomplished the MDF (e.g. May 10, 2022).
