The Pag-IBIG Member’s Data Form (MDF) is a document that you need to accomplish and submit in order to become a full-fledged member of the Home Development Mutual Fund (HDMF), or more commonly known as the Pag-IBIG Fund. The Pag-IBIG MDF contains a member’s personal details, addresses, contact information, employment history, and other relevant information. It also includes the 12-digit Pag-IBIG Membership ID (MID) Number as well as the Registration Tracking Number (RTN).
These days, registering as a member of the Pag-IBIG Fund can be done online. Prospective applicants need not fill out and submit the Pag-IBIG MDF because they can simply input their information on the Pag-IBIG online registration website. This is definitely easier and more convenient than the old method of manually filling out forms and personally bringing them to the Pag-IBIG office.
However, you can still use the Pag-IBIG MDF to apply for a Pag-IBIG number if you can’t (or don’t want to) register on the Pag-IBIG website. The Pag-IBIG Fund still accepts applications this way. Just download the Pag-IBIG MDF and print it on short bond paper, or you can get one at the Pag-IBIG office nearest you.
At the moment, there is no option to request your Pag-IBIG MDF online if you’re an existing member. All requests for a copy of the form must be performed personally at the nearest Pag-IBIG Fund office.
To request a hard copy of your Pag-IBIG MDF indicating your Pag-IBIG MID number, just visit any Pag-IBIG branch and bring the following:
You may download the Request Slip here. After downloading the slip, open the PDF file and print it (press Ctrl + P) on long bond paper (8″ x 13″). If you don’t have long bond paper, you may print it on short or A4 paper as long as you set the size to fit.
After accomplishing the Request Slip, go to the nearest Pag-IBIG branch and submit it along with a photocopy of your valid ID. You must also bring your original ID for authentication purposes.
Pag-IBIG branches are open from 8:00 AM to 5:00 PM, Mondays to Fridays (except for holidays).

Your requested Pag-IBIG MDF with Pag-IBIG MID number will be released on the same day. Your authorized representative may also claim your Pag-IBIG MDF by presenting an authorization letter and valid IDs of the requestor and the authorized representative.
Take note that the Pag-IBIG MDF that you will receive is a system-generated copy, not the original form that you submitted during membership registration.

For best results, you should print the form on letter size or short bond paper (8.5″ x 11″). You are required to submit only one (1) copy of the Pag-IBIG MDF.

If you’re not a Pag-IBIG Fund member yet, you can apply to become a member by accomplishing the Pag-IBIG Member’s Data Form (MDF) and submitting it to the nearest Pag-IBIG branch.
Select the appropriate option to indicate the type of membership coverage under Republic Act No. 9679.
List your legal heirs in accordance with the Laws of Succession, as provided in the New Civil Code of the Philippines (as amended). You can use another sheet if necessary.
Read the certification carefully, and then affix your signature and indicate the date when the Pag-IBIG MDF was accomplished.
